Medical Authorization Form and Release

I for myself and as the parent and/or legal guardian do hereby authorize Evolution Action Sports Training LLC to transport my child and/or ward to a doctor, hospital or other health care facility. I release Evolution Action Sports Training LLC, its owners, instructors, employees, agents and servants, from any and all liability for personal injury to me or my child and/or ward as the result of any negligence in transporting medical or hospital treatment or any delay in such transportation, selection or treatment. By signing this release, I acknowledge my understanding and acceptance of the following: Evolution Action Sports Training LLC has my permission to use its discretion in determining whether my child and/or ward requires medical attention and, if so, to use its discretion in transporting my child, selecting a health care facility and obtaining treatment for him/her. I HAVE READ THE MEDICAL AUTHORIZATION RELEASE AND RELEASE OF LIABILITY FOR PERSONAL INJURY AND I ACKNOWLEDGE MY UNDERSTANDING AND RESPONSIBILITY OF FOLLOWING THESE POLICIES AND PROCEDURES. Release of Liability

I do hereby release Evolution Action Sports Training LLC its owners, operators, instructors, employees and agents from any and all liability for personal injury to me or my child and/or ward as the result of any negligence arising out of or in the course of or in any way related to my or my child’s use of the facilities, equipment, apparatus or premises of at Evolution Action Sports Training LLC at 661 Pleasant Street, Norwood, MA. On behalf of myself and my child, I agree to indemnify and hold harmless the said claims, demands, costs, expenses and compensation arising out of or in the course of or in any way related to any personal injury to me or my child. By signing this release, I acknowledge my understanding and acceptance of the following: That all action sports require strength, agility and concentration and that it is solely my responsibility to determine that my child is in good health and good physical and mental condition before permitting my child to exercise, work out, receive instruction or perform. That all sports require twisting, turning, jumping, flexion, extension and rotation, which movements are often performed with considerable force and/or at considerable height and which can result in severe, permanent personal injuries, including, but not limited to, bruised, strained, sprained or torn muscles, tendons and ligaments, broken bones, derangements or dislocations of joints, concussion, brain damage, nerve and spinal cord injury, paralysis and death. That all sports require the use of apparatus and/or equipment, which may cause or contribute to serve, permanent personal injuries, such as those described above. Camp Payment Policies

EAST REQUIRES A CREDIT CARD TO BE ON FILE FOR ALL CAMPERS. If you prefer to pay by Check or Cash, please note that in the comments section when registering. EAST requires a 1/2 down non refundable deposit upon registration. The balance is due on the first day of camp. Your deposit must be received within 1 week of registration. If payment has not been received within 1 week of registration EAST will charge your credit card. Class Payment Policies

E.A.S.T. Customers can pay by cash, check or credit card. Your tuition is due on the first class of each month. As of September 2010 E.A.S.T. now requires all members to have a credit card on file. Your tuition is due on the first class of each month. If we have not received your payment by the first class of the month we will automatically charge the credit card that is on file and add $5. All credit card payments are now charged a $5 Convenience fee. You will be charged a $25 Fee for returned checks. I  authorize Evolution Action Sports Training Center to charge the credit card indicated in this authorization form according to the terms outlined above.

I understand that this authorization will remain in effect until I cancel it in writing. I agree to notify the business in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date.  (SEE Discontinuing for more) Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all-current and future service. I certify that I am an authorized user of this credit card and that I will not dispute the payments with my credit card company provided the transactions correspond to the terms indicated in this authorization form. Missing Class/Make Up Policy

Please contact the office at 781-762-1414 or info@EvolutionActionSports.com if you are going to miss class. Please make every effort to have good attendance. Make up lessons are for missed classes only, no refunds or credits will be given. Missed classes will not result in prorated tuition or refund. All make ups must be scheduled in advance through the office scheduled within the same month as the missed classes.

Discontinuing

You must put in writing and email info@evolutionactionsports.com 15 days before your last class. If we are not notified we will continue to charge your credit card that is on file. In your email please include: Parents Name, Students Name, Date of form, Date of last class that your child will be attending, Reason for leaving, and anticipated return date. If you do not notify us you will be charged a $35 Administration fee.

Reminder New Policies

A convenience charge of $5 will be added to all Credit Card transactions.

New Student Registration: (Yearly): $25 per student or $40 per family.

If your credit card on file, is not valid and payment has not been received than you will be also charged with a $15.00 late fee.
